Title V FFY 2008-2009 Annual Application/Report – Public Document
This document is a preliminary and abridged version of California’s FFY 2008-2009 Title V Block Grant Application and Progress Report. The Maternal Child and Adolescent Health Program (MCAH) of the California Department of Public Health and the Children’s Medical Services Branch (CMS) of the California Department of Health Care Services are pleased to release the draft application for public review and comment. Title V serves all pregnant women, mothers, infants, children, and adolescents, including children with special healthcare needs. The document presents information on the MCAH and CMS landscape in the state and data on performance measures and objectives.
2010 Title V Local MCAH Jurisdiction Needs Assessment Guidelines
The State MCAH Program, in collaboration with its 61 local MCAH jurisdictions and the Family Health Outcomes Project, gathers input from local communities to do a comprehensive analysis that describes local public health issues, priorities and needs. These guidelines, attachments and worksheets provide local MCAH jurisdictions guidance on structuring their local needs assessment report, the results of which will be included in the 2010 statewide needs assessment.

This document is the final version of California’s FFY 2007-2008 Title V Block Grant Application & Progress Report narrative. The Maternal Child and Adolescent Health Program (MCAH) and California Children’s Medical Services (CMS) coordinate the Title V Block Grant Program in California. Title V serves pregnant women, mothers, infants, children, including children with special healthcare needs (CSHCN), and adolescents. The document presents information on the MCAH and CCS landscape in the state and data on performance measures and health status indicators.
Title V Implementation Plan
The goal of the Title V Implementation Plan is to develop objectives and strategies for implementation at the State level that address the priorities identified in the 2005 Title V needs assessment process. The Plan is a roadmap for guiding MCAH Program activities during the next five-year phase of the Title V Maternal and Child Health Block Grant and builds on existing and emerging efforts. The strategies listed under each objective illustrate the general direction to be taken during the remaining years of the Title V 2005-2009 grant cycle.
2005 Title V Needs Assessment
As a recipient of the federal Title V Maternal and Child Health Block Grant, California is required to complete a statewide needs assessment every five years, which was completed in Federal Fiscal Year (FFY) 2004-05. This 2005 needs assessment resulted in the identification of priorities for the maternal, child and adolescent health population over the next five years.
